A Clinical Study of Liver Abscess

Abstract
A statistical study of liver abscess was done for 67 patients treated surgically from January, 1972 to December, 1981 at the department of surgery, Wallace Memorial Baptist Hospital.
The results were as follow
1) Of 67 cases of liver abscess, 19(28.4%) were amebic and 48(71.6%) were pyogenic.
2) Male Outnumbered female by ratio of 3.5:1, and high age incidence was 4 th and 5 th decade.
3) The locations of liver abscess were the Rt. lobe in 47 cases(70.1%), Lt. lobe in 10 cases (14.9%) and both lobes in 10 cases(14.9%), and 19 cases(28.4%) were multiple abscess.
4) Average days suffered from illness was 26.2 days and 74.6% of patient were visit hospital within 3 weeks after the onset of first symptom.
5) 37 cases(55.2%) had causative past history, and 14 cases(29.2%) of pyogenic abscess had previous history of hepatobiliary disease.
6) The most common clinical manifestations were pain, fever and chill, and tenderness over R.U.Q.
7) The abnormal findings in C.B.C. and L.F.T. were leukocytosis, serum hypocholesterole. mia, elevation of serum alkaline phosphatase and serum hypoalbuminemia.
8) Positive roentgenologic findings on admission were present in 61.5%.
9) Liver scans showed space occupying lesions in 79.6% and ultrasonogram showed cystic lesions in 87.5%.
10) Pus culture were performed in 52 cases, of which 36.5% was positive, and E. coli and staphylococci were most frequently found.
11) Open drainage was performed in 64 cases, and the transabdominal drainage was performed in 59 cases(92.2%)
12) The incidence of complications was 31.3% in the pyogenic and 42.1% in the amebic abscess.
13) The overall mortality rate was 9.0% and sepsis was the most frequent cause of death.
